PHM 2021 Virtual will be a live virtual meeting from August 4 - 6, 2021. The meeting time zone is Eastern Daylight Time. The majority of the content will be live and pre-recording your presentation will not be required unless otherwise noted.

About Forms Due
Disclosure/Conflict of Interest Form
Each faculty member will receive a conflict of interest disclosure form from the AAP automated system. All conflicts must be disclosed by June 4.
Faculty Agreement Form
All lead faculty must fill out a Faculty Agreement Form to grant recording permissions and to attest that educational materials are accurate and to adherence to ACCME standards to provide CME to attendees. Lead presenters will log into the PHM submission system to upload their form. There are three different submission systems designated by your type of submission. You'll need your submission number and password to access it. If you do not have your login information, it can retrieved from the login page.
All presentations slides must be reviewed for conflicts and accuracy by a CME chair to provide CME to attendees. Lead presenters will log into the PHM submission system to upload their slides. Guidelines can be found in the Presentation Preparation tab below.
Poster guidelines, tips and information can be found in the Presentation Preparation tab below. Pre-upload is not required.
The primary leader for Clinical Quick Hits, Mini-plenaries, PHM stories and workshops receive complimentary registration. An email was sent with a code to claim the complimentary registration. If you are unable to locate this unique code, email email@example.com.
All co-leaders and presenters must be registered to confirm participation and gain access to the virtual platform.
Our virtual meeting platform is Pathable. This platform utilizes Zoom so most of its functionality will be available for your session (polling will be available directly through the virtual meeting platform). Speaker training will be hosted on July 7 from 5 pm – 6 pm EDT and July 16 from 12 pm – 1pm EDT.
If you are not available for either training, the recording will be made available to you. During this time, we will also review the technical support available to you during your session.
