Keep the Pressure On: A Pediatric VTE Risk Assessment and Prophylaxis

Summary:
At our institution we created a VTE risk screening tool and prophylaxis order set in the electronic health record. This evidence based tool was developed by a multidisciplinary team. We have achieved close to 80% reliability with our VTE prevention bundle and seen a significant decrease in the number of VTE events (2011: 7 VTE events, 2012: 4 VTE events, 2014: 1 VTE event to date). We intend to share our experience in developing and successfully implementing the tool. Will discuss the risk factors for VTE and the rationale for risk stratification and prophylaxis. We will also discuss the challenges and barriers for appropriate utilization and successful implementation of the tool. We will share our experience with using the framework of the Model for Improvement and multiple PDSA cycles we ran for the development, spread and successful implementation of the tool. Attendees will have access to our tool and will be equipped with some quality improvement tools so that they can implement in their own facilities.
